Our new address is: 104 Ashourian Avenue Suite 105 St. Augustine, FL 32092 The new St. Johns location of Brooks Rehabilitation Outpatient Services is located just east of St. Johns Parkway at the Forum. Our new location provides comprehensive services including physical therapy, occupational therapy and speech therapy for children and physical therapy for adults.
